Book an appointment by following the process below:
REFERRAL FORM
Use your phone to scan or photograph your Referral Form before you start. You will be prompted to upload the image at Step 3.
Click Button
Located on the right hand side of this page, select the button and a new Form will appear on the side.
Complete Form
You will be guided through the process by the Avatar. You may disable the voice at any time.
SUBMIT
Upon submission of the completed form your Request will be sent to our Booking team.
SMS
24 hours before your appointment you will receive an SMS to reconfirm your requested appointment time, date and location.
OR if USE THIS ALTERNATE FORM FOR SPECIFIC REQUESTS
Once we receive your appointment request, one of our team here at The X-ray Group will call to confirm the date and time of your appointment, along with confirmation of your personal details.
